Home-Base Roadmap
FROM PAPER TO PRACTICE

You know your child’s goals, but you aren’t sure how to make them "real" outside of a therapy session. The Strategy Blueprint provides the professional translation you need to turn IEP jargon into a functional home plan. We do the heavy lifting of analyzing the clinical recommendations so you can focus on being a parent.
Services Include
IEP Deep Dive: A comprehensive review of your child’s current OT, PT, SLP, and behavioral goals.
The Generalization Guide: A 3-page custom roadmap mapping school goals to your specific home routines (e.g., "Instead of 'pincer grasp practice,' try this during Saturday morning cereal").
Virtual Action Session: A 60-minute 1:1 strategy call to walk through your plan and answer your "How do I actually do this?" questions.

The Collaborative Bridge offers consistent, encouraging support as you practice strategies at home—helping you notice what’s working, adjust when things feel stuck, and build confidence over time. This tier is rooted in reflection, coaching, and shared problem-solving—without changing your home setup or adding new systems.

Integrated Life is designed for situations where consistency—not effort—is the missing piece. This tier focuses on aligning goals, language, and expectations across home, school, and therapy so skills can generalize more efficiently and with less day-to-day friction.
Rather than adding more strategies or increasing parental workload, this level of support centers on coordination, clarity, and intentional follow-through.

All planning, coaching, and implementation support included.
Home Context Analysis
A structured in-person visit (where available) or virtual review to understand how existing routines, spaces, and transitions support skill carryover—informing targeted, practical recommendations.
Interdisciplinary Coordination
Facilitated collaboration with school teams and/or private providers to align terminology, prompts, and expectations across environments.
Extended Implementation Support
Ongoing, scheduled consultation and coordination to support consistency over time and ensure strategies are implemented as intended.
