Transition Blueprint

TURNING STRENGTHS INTO A STRATEGIC PATH FORWARD.

The Transition Blueprint is a strategic, customized roadmap built around your child’s strengths, interests, and long-term goals. Instead of piecemeal decisions, you’ll gain a comprehensive plan that aligns school services with real-world outcomes.

The process includes:

  • An in-depth student and family intake interview

  • Expert review of existing transition assessments and documentation

  • Development of a personalized Transition Blueprint

Your Custom Transition Blueprint includes clear goals and timelines across three critical areas:

  • Post-Secondary Education: college, vocational programs, trade schools, or certifications

  • Employment: job exploration, transferable skills, job shadowing, and agency supports

  • Independent Living: financial skills, transportation, community engagement, and daily living supports

Final Step:

  • A 90-minute strategy session to walk through the Blueprint, answer questions, and outline next steps for implementation

This service gives families peace of mind—and a plan that finally makes sense.
